Why Urethroplasty has chance where OIU or urethral dilatation has failed or is bound to fail?

The raw area that cutting a stricture with OIU creates has a tendency to narrow down again because the scar heals with further scarring. In short or soft strictures however, the inner lining of the urethra(the mucosa) covers the raw area before the scar can re form. In larger strictures, this does not happen and the scar wins the race

What a substitution Urethroplasty does, in essence, is to cover the raw area with lining from some other tissue ( graft or flap) which intends not to allow the scar to come back Mostly it wins, sometimes it doesn’t
